In order to do so, you need to “Crush 20 enemies with heavy objects in a single playthrough.”

The easiest way to do this is during The Vanishing Act level when you first learn about how to drop anvils. If you time it right, you will be able to crush two enemies.
Afterwards, reload from your last save and then drop the anvil again. Rinse and repeat this process to unlock Spike-D 16 TONS.
